Riociguat is a stimulant of soluble guanine cyclase (sGC), an enzyme in the cardiopulmonary system and a receptor for nitric oxide (NO).When NO binds to sGC, enzymes catalyze the synthesis of the signaling molecule cyclic GMP (cGMP), which plays an important role in the regulation of vascular tension, proliferation, fibrosis and inflammation.Pulmonary hypertension is associated with endothelial dysfunction, impaired nitric oxide synthesis, and inadequate stimulation of the no-sgc-cgmp pathway.Riociguat has a dual role model.It synthesizes GC to endogenous NO by stabilizing NO-sGC binding.Riociguat also stimulates directly through different binding sites independent of NO.Riociguat stimulates the no-sgc-cgmp pathway and leads to increased cGMP generation with subsequent vasodilatation.
