Telmisartan interferes with the binding of angiotensin II to AT1 receptors in vascular smooth muscle and thus inhibits the
vasoconstricting action of angiotensin II. Additionally, telmisartan is a selective modulator of peroxisome proliferator-activated receptor gamma (PPARγ), an essential regulator of insulin and glucose metabolism. Therefore, telmisartan is useful for studying hypertension.
Function & Application
Telmisartan is a new antihypertensive drug, a specific angiotensin II receptor (AT type I) antagonist, for the treatment of essential hypertension. High affinity binding between angiotensin II receptor and AT I receptor subtype (known site of angiotensin II). Ti Mi Chatain did not have any agonist effect at the position I of AT I, and was selectively bound to AT I receptor, which had a long-lasting binding effect.
There is no affinity for other receptors (including AT2 and other AT receptors with fewer characteristics). The function of these receptors is unknown, and the effect of the receptor over stimulation may be unknown due to the increased level of angiotensin II induced by telmisartan.
Telmisartan does not inhibit the plasma renin nor block the ion channels. It does not inhibit angiotensin converting enzyme II, and this enzyme can also degrade the adverse effects caused by the increase of bradykinin. 80mg telmisartan in human body can almost completely inhibit angiotensin II induced elevated blood pressure. The inhibitory effect lasted for 24 hours and could still be detected at 48 hours.
The antihypertensive effect was obvious within 3 hours after the first dose. The maximum antihypertensive effect was achieved 4 weeks after treatment and maintained in long-term treatment. After a few days of treatment, the blood pressure gradually returned to the level before treatment without rebound hypertension.
In the direct comparison of the clinical trials of two kinds of antihypertensive drugs, the incidence of dry cough in the treatment group was significantly lower than that in the angiotensin-converting enzyme inhibitor group.
